Complete Your Forms in 3 Easy Steps:
- Review & Update: Check emergency contacts, complete the income section, and confirm education levels.
- Submit: Click "Finish and Submit" at the end.

- Effective Date: Date completing form
If you would like to decline to provide medical insurance information, please follow the directions below:
- Should you decline to provide medical insurance information, click the “Add record button” and select “Decline to provide” under insurance type.
- Data Confirmation will ask for a date as well (required field).
- Enter the date you are completing the form as the date you “Declined to provide” the medical insurance information.
Please complete the annual Parent Data Confirmation for each of your students today, Friday, September 4!
